day at a time. POSITION SUMMARY Our Nurse Practitioners (NPs) and
Physician Assistants (PAs) are essential contributors to our
organization’s success, aiding our partners in improving the
quality of life for their residents. Key responsibilities include:
Evaluating patients’ medical and mental health requirements and
developing a care plan that proactively addresses their conditions
and overcomes barriers to care. Ensuring adherence to regulations
set by local, state, and federal agencies concerning the clinical
services provided. Prescribing medications and arranging lab tests,
diagnostic procedures, and consultations. Tracking patients’
adherence to treatment plans and adjusting those plans as
necessary. Collaborating with RN case managers as part of a
cohesive care team. WORK EXPERIENCE, CREDENTIALS, AND EDUCATION
Required licensure as an FNP, AGNP, AHACNP, or PA. Graduation from
an accredited APRN or PA program. A minimum of 3 years of
experience in clinical nursing or rehabilitation with geriatric
populations. Experience with Electronic Health Records. Proficiency
in Microsoft Office applications, including Word, Outlook, and
Excel. SUPERVISORY RESPONSIBILITIES May be tasked with providing
